Dear All

I took medibase (http://www.vikingcomputers.com.au/medibase/) for a spin
tonight and was really quite impressed for a first beta.

I just ran the demo so have no idea how good the MD import data facility
was. I ran both the interbase server and medibase client on an old NT
machine with 64 meg RAM which is obviously well below specs.

Most MD facilities are available in the usual tabbed format and the
response on my machine was fast (as to be expected) even with multiple
patients open. Only the MIMS PI struggled and even this was faster than
under MD.

I particularly liked the fact that the Consultation notes screen can be
enlarged and shrunk with a few button clicks or varied in size and shape
with the scroll bar drag in both vertical or horizontal directions.
Initially I thought the history taking facility was extremely limited
due to the lack of autotext generation. However, I eventually found it
under Tools | Options | Autotext. Some help on how to get all my old
shortcuts out of MD would be useful.

I managed to trigger a few access violations by clicking things I did
not have but this did not crash the program and simply threw up an error
dialogue box.

Those keen on templates will be pleased with the flexibility available
to create your own and if you like diagrams they are are large and pretty.

The history pic list was good in that it allowed string selection on any
part of the diagnosis, e.g. "myo" will bring up esophagomyotomy as well
as myocardial infarction et alia. I am not sure what coding system they
are using but I certainly hope the pick list will be velocity coded.

Printing seemed a little slow but this may well have been due to my
machine and setup (viz default printer not connected, subspec RAM, etc.)
The email facility worked with a copy and paste so now all that is
needed is to send it to a gpg, Argus or Epitmail pipe.

I didn't come across a test pathology import file but will look a little
futher in due course. Requests and Results are on adjacent tabs in the
ToDo tool and I was not sure (in the absence of a test pathology file)
if there was some attempt to link one to the other.

I understand that medibase will import from PracSoft and similar billing
programs using the MD patients.in format. There is a tool to do this on
the menubar and I presume it can be run in the background as an
independent facility.

In summary Medibase looks like the full deal, running on a sturdy
Interbase client server platform with either a linux or windows backend
and the possibility of a linux client in the future. I also understand
that work on a billing package is scheduled for a later date.

Medibase is certainly worth a look if you are planning to move to the
next generation of computer software.

I have had a play yesterday and for the record Mandrake 9.1 is a very
nice operating system. It is far less kludgy than it used to be and is
now almost approaching Windows XP in terms of the quality of its
appearance. The quality and amount of software available remains truly
amazing.

looks good, but here are my thoughts on internal modems.

-I prefer the external variety, ethernet, not USB.
-If something goes wrong with the card, you will need to take you server down.
-The external modem can be used with anything, Linux, Mac, Windows, etc.
-never any problems with drivers as you only need to deal with network cards.

That's just my preferences, YMMV

It looks like there is a recent thread on SLUG about this. Apparently
Pioneer Computers http://www.pioneercomputers.com.au/  can provide one
but even getting one there looks like hard work.

I must admit to being surprised as to the virtual impossibility of
obtaining a laptop without Windows installed. I guess that's what
happens when there are relatively few manufacturers who have to keep the
major software supplier happy.

MedPlexus Inc. have signed a letter of intent to make available an
open-source electronic health record (EHR) for family practice and other
office-based medical specialties. The letter of intent serves as a
framework for negotiation of a definitive agreement between MedPlexus
and a proposed not-for-profit foundation planned for establishment by
the AAFP and other medical associations. Upon successful completion of
the definitive agreement, the foundation will use MedPlexus' software as
the core architecture for its open-source EHR. As envisioned, the
project will likely integrate software from other companies as well, to
create a robust and flexible set of electronic tools for use in the
outpatient care setting.

EHRs are essential in increasing the quality of health care and
improving patient safety. "An EHR should contain detailed clinical
notes; prescription ordering and management capability; a secure
messaging system; lab and test results reporting; evidence-based health
guidelines; secure and confidential patient access to health records;
public health reporting and tracking system; mapping to clinical and
administrative standard code sets; and the ability to interface with
leading practice management software," said David C. Kibbe, M.D.,
director of health information technology at the AAFP. "The open-source
EHR that we are proposing will be designed to run on PCs, Macintosh,
Linux, UNIX, Palm and PocketPC hand-held devices."

"It is exciting to collaborate with a trusted, forward-looking
organization such as the AAFP on a project that will improve the
healthcare system for all involved - patients, providers and payers,"

Hi,

as people are always looking for an outlook replacement, shared calendar, etc,
I thought that this may be of interest to some of you. Mitch Kapor of Lotus
1-2-3 fame has started a PIM called Chandler. It will be open-source and run
on Linux, Mac and Windows. It does sound interesting even if it is only prior
to the 0.1 release.

Had a quick look at the web site. This seems to be a good tool to help you
build firewall rules. I guess that's if you want to roll your own firewall.
If you use something like SME Server, Smoothwall, IPCop and others, they
already come pre-configured pretty securely. For those systems, I'm not sure
of the usefulness of firewall builder. For some complex network
configurations it would be a very useful tool indeed.

I have seen similar CLI tools before, but a GUI seems to work better for some.

Never had that problem as far as I know, but I have had the opposite
problem - the path results that wouldn't go away.
It was my fault - I thought I would do what all self-deluded network
administrators would do and fiddle the permissions on my MDW data
directory (separate to the MDW program directory) to prevent anyone
but me from actually deleting any data files. Win 2000 server does
have fine grained permissions in this respect and I did make the
change one morning before work. I made the change to the whole
directory.
No problem was evident until I downloaded the pathology using MD mail
then checked the holding file. Every time I performed a "check
pathology results" the same, previously checked results would
reappear in the holding file.
When I looked in the data directory, sure enough all the .qml
and .snp files were still there.
No problem, I thought I would just tell MD mail to deposit the
decrypted files in another directory with looser permissions.
Unfortunately after some rather frantic looking I came to the
conclusion that the only way to achieve this would be to break into
HCNs head office, steal the source code, modify it with a new target
directory and recompile.
I realise that this has to be wrong but I just could not find a way
around it - any help would be appreciated.

I have recently had some path results disappear between after being
commented on in the holding file and before linking to the patient file.
In one instance three of four results went in but the fourth went AWOL.
It appears not to be a user problem.

MD version 2.72b. dbfs=1.51 gig.

I know HCN will tell me to upgrade first but has anyone else seen this?

Just to follow up on the slow network browsing on a win2000 server.

I have fixed this by making the workstation a static IP. It was originally a
DHCP from an ADSL internet router (Dlink dsl 504).
The sql pracsoft version would not install on the workstations and this was
fixed by making the workstations fixed ip (have not had the win2000 server box
do the DHCP).
Once this was done the slow browsing of the folders on the server was fixed.

Thee e-smith servers (one of which I am planning to morph into an
ipcop), one  NT box and one MS TS. The MS boxes have monitors but they
are usually off. The noise is probably as big a factor.

We need to tidy up this mess. Hence the rack.

if you are looking for rack mount servers or if anyone is looking for a new
server of any sort then Dell are clearing out new servers with 12 month warranty
via www.graysonline.com.au See the section on online auctions.  They have some
servers and workstations that look really great.
